My team has a habit of being pretty far out into the open and doesn't LoS or run around something when focus'd and I'm being cc'd.
This is your first problem, you and your ele shouldn't have to leave your base unless hes pushing up for a hex on the healer. The lock can push out to fear because he can always port back if his position is compromised.
Another thing you should stress is gateway usage. It's a really powerful cooldown with a somewhat low cooldown. I find that once a team pushes into you and commits, especially cleaves, its best to use gateway as a team to open up some distance so you can wizard them down while they waddle across the map. If you port early, you can save yourself from using alot of defensive cooldowns, leading to being able to prolonge the game longer until you can build up enough pressure to kill something.
